The Outpatient Heart Failure Clinic at Northside Hospital assists in the immediate care provided to heart failure patients in the local community. Staffed by a board-certified acute care nurse practitioner, the clinic acts as an extension of our local healthcare providers.
The goal of Northside Hospital's Heart Failure Clinic is to help bridge the gap between hospital discharge and the patient being seen for a follow-up appointment with their primary physician or cardiologist. Clinic services can also be available for those physicians and clinics that encounter a large volume of patients, which can sometimes make it difficult to schedule a heart failure patient within a short timeframe.
Services provided in the clinic:
- Rapid hospital follow-up (within 24-72 hours post-discharge)
- Comprehensive heart failure education for both the patient and his/her family
- Medical therapy consistent with evidence-based practices
- Emergent intravenous diuretic therapy
The Heart Failure Clinic is located in the Medical Office Building on the north end of the Northside Hospital campus.
